Online Forms and Consultations

Thank you for using the online consultation option, as this allows you to access help in a timely manner and will take less time than it would take for you to phone us.

All our online forms and consultations can be found on SystmConnect.

Our prescription team are happy to receive your prescription requests via the “Medication request” option.

Our team also prefer to receive your Fit note requests (also known as a Med3 or sick note) this way, as this asks all the questions that are needed for us to produce this in a timely manner for you.

No login details or account is needed, but you can use your login for the NHS App or SystmOnline to pre-populate some of your information if you wish to.

The aim is that online requests will be accepted throughout our usual opening hours, Monday to Friday (excluding bank holidays). These requests should be things considered routine and this is not for urgent or emergency care.

FAQs

1. Can I submit an online consultation at any time

You are welcome to submit an online form at any time. New, routine online requests will be processed during our usual opening hours, Monday to Friday (excluding bank holidays). We aim to respond to these requests within 24hrs. Forms submitted as part of the annual review process (e.g. Asthma Questionnaires) will be processed within a week.

2. Can I put a prescription request in at any time?

Yes.

3. How do I make a request for someone else?

You can complete a request on behalf of someone, either as their relative or friend, or healthcare professional.


Firstly, pick the type of request (New Condition, Follow Up, etc), then pick the nature of the problem. Once you have completed the problem page, the system then asks who you are completing the consultation for.
The form will change to ask for your information and the information of the person you are completing this for.

4. Can you show me how to do this?

Yes, our reception team will show you how to complete an online consultation if you would like some support. Bring along your phone or electronic device that you use, and one of our team will be able to sit with you and show you the steps.

5. What if I haven’t got a Smartphone or internet access?

If you can’t complete an online form, or would prefer to contact us over the phone you are welcome to do so.

Setting up an NHS account

NHS accounts are not run by the surgery team. To set up your NHS account for the first time, you will need to set up an NHS login. Read about how to set up NHS login (nhs.uk).

Once you have set up your NHS login, you can log in to your NHS account or download the NHS App onto your phone. This will allow you to access SystmConnect through the app.
